PATIENT RIGHTS AND RESPONSIBILITIES


As a patient, you have the right to:

As a patient, you have the responsibility for:    

1. Considerate, respectful care at all times under all circumstances with recognition of your personal dignity, civil rights & religious opinions.

2. Personal information privacy, within the law.

3. Information concerning your diagnosis, treatment, and prognosis, to the degree known.

4. Confidentiality of records & disclosures. Except when required by law, you have the right to approve or refuse the release of records.

5. To participate in decisions involving your health care, unless contraindicated by concerns for your health.

6. Make decisions about medical care, including the right to accept or refuse medical or surgical treatment and the right to initiate advance directives.

7. Information concerning implementation of any advance care directive.

8. Access to treatment regardless of race, color, sex, national origin, religion, handicap or disability. (FCASC adheres to all federal & state rules, regulations & policies to promote a non-discriminatory environment for all of our surgical guests.)

9. Know the identity & professional status of individuals providing services to you.

10. Report concerns of the quality of services during the time spent at the facility & receive follow-up on your comments.

11. To obtain assistance in interpretation for non-English speaking patients.

12. To receive care in a safe setting.

13. If a patient has been deemed legally incompetent, the rights of the patient are exercised by the person appointed under state law on the patient’s behalf.

14. Be free from abuse, harassment, discrimina-
tion, and reprisal.

15. Patients are provided appropriate privacy at
check-in and in evaluation and treatment areas.

1. Providing, to the best of your knowledge, accurate and complete information about your present status and medical history and reporting any changes to the appropriate Physician(s).

2. Following the recommended treatment plan by the Physician in your case.

3. Providing an adult to transport you home after surgery, and an adult to be responsible for you at home for the first 24 hours after surgery.

4.Indicating whether you clearly understand a contemplated course of action and what is expected of you.

5. Your actions if you refuse treatment, leave the facility against the advice of the Physician, and/or do not follow the Physician’s instructions relating to your case.

6. Assuring that financial obligations of your care are fulfilled as expediently as possible.

7. Providing information about and/or copies of advance directives.

8. Being respectful of all staff and guests.
